Steps to Service
- INTIAL CALL – Once we’ve received your information, we will have short 15 minute call to answer any questions or concerns and set up a time to meet in person.
- IN-HOME CARE ASSESMENT – Here we will answer all your questions, and formulate a Customized Care Plan base on your needs.
- START OF CARE – We will come out with our caregiver on the first day of care to go through the Care Plan together and introduce them.
- CLIENT VISIT – Once we have begun care, the Registered Nurse will pop in at times to see how things are going and determine if we need to adjust the Care Plan.
Plans we currently accept:
We accept most Medicaid Managed Care Plans and are licensed by APD (Agency for Persons with Disabilities) to provide services to individuals on a Medicaid Waiver program (iBudget Waiver or CDC+).
